CANCELLATION POLICY

Appointment Cancellation & No-Show Policy At Well Life Integrated Medical, we are committed to providing exceptional, timely care for every patient. When you schedule an appointment, that time is reserved exclusively for you. Last-minute cancellations and missed appointments prevent other patients from receiving care and affect our ability to maintain efficient scheduling. Appointment Cancellation If you need to cancel or reschedule your appointment, we kindly request at least 24 hours' notice. You may cancel by: Calling the office during business hours Leaving a voicemail after hours Responding to your appointment reminder (if applicable) Voicemails left before the 24-hour deadline will be honored. Late Cancellation Appointments canceled with less than 24 hours' notice will incur a $25 Late Cancellation Fee. No-Show Policy Patients who fail to attend their scheduled appointment without notifying the office will be considered a No-Show and will be charged a $25 No-Show Fee. Late Arrivals Patients arriving more than 15 minutes late may be asked to reschedule if there is insufficient time to safely complete the visit. This may be treated as a late cancellation and may be subject to the $25 cancellation fee. Medication Visits Medical weight loss medications, including compounded GLP-1/GIP therapies and other wellness treatments, require ongoing medical supervision. Missing follow-up appointments may delay medication refills, dose adjustments, or future prescriptions until the patient has been appropriately evaluated by the provider. Card on File A valid debit or credit card is required to remain on file for all patients. By scheduling an appointment, you authorize Well Life Integrated Medical to charge applicable cancellation or no-show fees in accordance with this policy. Repeated Missed Appointments Patients with: Three (3) no-shows, Three (3) late cancellations, or A combination of repeated missed appointments within a 12-month period may be required to prepay future appointments or may be dismissed from the practice at the provider's discretion. Emergencies We understand that emergencies and unexpected illnesses occur. Fees may be waived at the sole discretion of the provider for circumstances including: Medical emergencies Hospitalization Severe illness Death in the immediate family Dangerous weather or natural disasters Acknowledgement By scheduling an appointment with Well Life Integrated Medical, I acknowledge that I have read, understand, and agree to abide by this Appointment Cancellation and No-Show Policy.
